I feel like a total failure, i keep telling myself ill make it, i exercise to get my mind off of it, i play guitar, watch youtube, but i lay in bed and 5 minutes later im looking at softcore then full on hardcore porn. I feel pathetic. I cant overcome the urges, it takes over my whole body and i just say "FUCK IT" and go all out in it... I need real help i have mild PIED but im afraid if i keep relapsing ill never fix it.

First of all, relax men, we all felt that at the start. In my personal case I did relapse twice after my reboot. That 2 times were after a week.
Now im on the good reboot one. Last week i did 2 months, so listen:

So, my advice and my experience tell that the first 2 weeks are the worst ones. I actually also felt that I could not reboot and stop masturbating and watching porn but its all in your mind bro. Just relax and keep it controlled. You must understand is a long process, so do not despair, and, if you feel the need to masturbating or watch porn think about how frustating is stay in front a beautiful girl that is desiring to have sex with you and you can not because you cant get erected.

Once you pass the first 2 weeks you will feel really really relaxed, you will start feeling happier and you will not have the need to watch porn or masturbating. Even you start avoiding it because you know is not what your body wants.

So bro, stay strong, especially the mind. The mind is what controlls all.

Don't use Youtube as a replacement for porn. It's a visual medium, like porn. It excites your brain, like porn. It gives you dopamine hits, like porn. It presents random content in the form of ads and video suggestions that can display semi-erotic content. In short, it's probably keeping your brain in a semi-aroused state and making it want more. This makes you more vulnerable to a relapse with porn. I think decreasing internet use in general is very important for porn addicts trying to quit. Sitting in from of a computer or phone, staring at a screen is a much too similar way to spend your time. How is your brain supposed to feel there is a real change occurring if you spend the same amount of time looking at your devices? It's functioning as a porn substitute. Part of what a reboot is is a return to the real world. The more time spent away from screens and doing real activities the better. Maybe start reading books in your downtime.

"but i lay in bed and 5 minutes later im looking at softcore then full on hardcore porn"

This isn't the problem I had with relapsing because i'd typically masturbate to porn on the computer in my home office rather than in bed. I found rearranging the office (unexpectedly, and done for a different purpose) helped enormously with the triggering behavior. I'd sit down at my computer and masturbate, but rearranging the office and moving the desk to face a wall instead of the windows reduced the temptation significantly.

If you can rearrange the bedroom as well, put the bed on a different wall, maybe get new linen, swap posters and things out, move other furniture around, whatever, that might help but not making it the "place you do this".

And yeah, it will get worse if you keep doing it in all likelihood.
